This position is responsible for providing consultations and education that is designed to promote the maximum level of wellness and independence for the patient with diabetes mellitus in multiple settings, acute and outpatient.

Two years acute care medical care experience required with a current CA Clinical Social Worker licensure. Effective problem solving skills. ability to relate variety of health professionals in different settings. Good interpersonal and communication skills with a commitment to group goals and teamwork.
